Medically reviewed by Scott Bradley Burris, MD. Reviewed May 2026.

Personalized semaglutide and tirzepatide formulations from licensed compounding pharmacies, with branded FDA-approved options available. Lab-guided. Physician-led. Cardiometabolic-focused.

At Hartwater Wellness, we believe weight loss should be science-backed, medically supervised, and tailored to your unique metabolic profile. GLP-1 receptor agonist therapy represents one of the most significant advances in medical weight loss and metabolic optimization, not because it's a quick fix, but because it works by addressing the underlying biological drivers of weight gain, appetite dysregulation, and cardiometabolic disease. Hartwater Wellness provides GLP-1 therapy within a comprehensive, concierge program designed for safety, effectiveness, and long-term success, with medications sourced exclusively from trusted and reputable pharmacies

GLP-1 medications work with your body’s natural biology to regulate appetite, slow digestion, and support healthy blood sugar levels. The result: effective, sustainable weight loss that goes beyond diet and exercise alone. While individual results vary, many patients lose 10–15% of their body weight within the first year of therapy.

While GLP-1s are most commonly associated with weight loss, GLP-1 medications have shown a surprisingly broad range of other health benefits. Clinical studies have found that GLP-1s can reduce the risk of major cardiovascular events, improve blood sugar control and diabetes prevention, and significantly improve liver health.

How GLP-1 Works in the Body

WHAT IS GLP-1 THERAPY

The Science Behind GLP-1 Receptor Agonists

GLP-1 therapy targets a fundamental problem: dysregulation of the hormonal signals that control hunger, fullness, blood sugar, and metabolism. Unlike older diet pills that relied on stimulants or appetite suppression alone, GLP-1 receptor agonists work with your body's natural physiology.

What Is a GLP-1 Receptor Agonist?

GLP-1 (glucagon-like peptide-1) is a naturally occurring hormone your intestines produce in response to nutrient intake. GLP-1 receptor agonists are medications that mimic this hormone, triggering a cascade of metabolic signals: they slow gastric emptying (keeping you full longer), reduce appetite through hypothalamic signaling, enhance insulin secretion in response to meals, and improve glucose homeostasis. In clinical trials, GLP-1 medications produce 15–22% body weight reduction and meaningful cardiovascular event reduction.

The Science Behind GLP-1 Receptor Agonists

GLP-1 doesn't work by numbing your brain or forcing willpower. Instead, it resets three key metabolic levers:

  • Satiety and Appetite Suppression: GLP-1 activates receptors in the hypothalamus and vagal afferent neurons, creating genuine signals of fullness. You eat less because you actually feel satisfied sooner—not because you're trying hard.

  • Glucose Regulation: GLP-1 enhances glucose-dependent insulin secretion, meaning your pancreas releases insulin only when blood glucose rises. This stabilizes blood sugar and reduces insulin-driven fat storage.

  • Gastric Emptying Delay: By slowing the rate at which your stomach empties food into the small intestine, GLP-1 extends the window of nutrient absorption and prolongs satiety signals.

  • Body Composition Preservation: While older weight-loss methods stripped both fat and muscle, GLP-1 therapy—combined with adequate protein and resistance exercise, preferentially preserves lean muscle mass, protecting your metabolic rate and functional capacity.

The clinical evidence is robust. The STEP trials (semaglutide) and SURMOUNT trials (tirzepatide) enrolled thousands of participants and demonstrated that GLP-1 therapy produces meaningful, sustained weight loss across diverse populations, with acceptable safety profiles when used under medical supervision.

GLP-1 Beyond Weight Loss: Cardiometabolic and Systemic Benefits

The most compelling evidence for GLP-1 extends beyond weight loss. The SELECT trial, published in 2024, showed that semaglutide reduced major adverse cardiovascular events (MACE: heart attack, stroke, cardiovascular death) by 20% in individuals with obesity, even independent of weight loss magnitude.

Why? GLP-1 provides systemic metabolic benefits:

  • Reduced Visceral Fat: GLP-1 preferentially mobilizes harmful visceral adiposity (fat around organs), which drives inflammation and cardiometabolic disease.

  • Blood Pressure Reduction: Users typically experience 4-8 mmHg systolic reductions, mediated by weight loss, salt sensitivity reduction, and improved endothelial function.

  • Improved Lipid Profiles: GLP-1 reduces triglycerides, LDL cholesterol, and improves HDL patterns independent of weight loss.

  • Reduced Hepatic Steatosis: Fatty liver disease improves as both liver fat and systemic insulin resistance decrease.

  • Anti-inflammatory Effects: GLP-1 may reduce systemic inflammation markers, with downstream benefits for immune aging and disease resilience.

At Hartwater Wellness, we measure these markers, not just weight. We track HbA1c, triglycerides, inflammatory markers, and blood pressure alongside body composition changes using DEXA or bioelectrical impedance analysis (BIA). This ensures your GLP-1 therapy is delivering the full spectrum of metabolic benefit.

Semaglutide vs. Tirzepatide: What's the Difference?

GLP-1 MEDICATIONS

There are currently two classes of GLP-1 receptor agonists available for weight loss: semaglutide and tirzepatide. Both are effective; they differ in mechanism, potency, and trial outcomes.

Semaglutide

Semaglutide is a GLP-1 receptor agonist, meaning it activates the GLP-1 receptor pathway exclusively. It was originally developed for type 2 diabetes management (Ozempic®) but showed such dramatic weight loss effects that the FDA approved a higher-dose formulation for chronic weight management (Wegovy®).

In the STEP trials, semaglutide produced average weight loss of 15–18% at the 2.4 mg maintenance dose, with approximately 80% of participants achieving clinically significant (≥5%) weight loss. Participants also experienced reductions in cardiometabolic risk factors and improvements in quality of life metrics.

Advantages of Semaglutide:

  • Well-established safety and efficacy data from decades of diabetes use

  • Approved dosing pathway: gradual titration from 0.25 mg reduces GI side effects

  • Available at Hartwater Wellness as a cash-pay service with transparent monthly pricing

  • Lower cost than tirzepatide at therapeutic doses

Considerations:

  • Modestly lower weight loss ceiling compared to tirzepatide

  • Gastrointestinal side effects (nausea, vomiting, diarrhea) most common during titration

Tirzepatide

Tirzepatide is a dual GIP/GLP-1 receptor co-agonist. This means it activates not only the GLP-1 receptor, but also the GIP (glucose-dependent insulinotropic polypeptide) receptor pathway. This dual mechanism appears to produce superior weight loss outcomes.

The SURMOUNT trials demonstrated average weight loss of 20–22% at the 15 mg maintenance dose, with some participants achieving reductions exceeding 25%. Tirzepatide also showed robust improvements in blood pressure, lipid profiles, and inflammatory markers.

Why the Dual Mechanism Works Better:

  • GIP activation enhances insulin secretion and may increase energy expenditure, amplifying semaglutide's effects

  • Synergistic appetite suppression through dual pathway activation

  • Improved glycemic control even in non-diabetic populations

Advantages of Tirzepatide:

  • Highest weight loss efficacy: 20-25% average reduction

  • Superior cardiometabolic improvements

  • Newer mechanism: ongoing research may unlock additional benefits

Considerations:

  • Shorter clinical history than semaglutide (approved for weight loss in 2023)

  • Slightly higher GI side effect incidence in early trials

  • Higher cost, available with transparent cash-pay pricing at Hartwater Wellness

Which GLP-1 Medication Is Right for You?

Both medications are highly effective. The choice depends on your clinical profile, goals, budget, and preference for side effect profiles. At Hartwater Wellness, we'll review your metabolic workup, medical history, and objectives to recommend the best-fit medication.

Consider semaglutide if:

  • You prioritize lower cost and established safety data

  • You have a history of GI sensitivity and prefer gradual titration

  • You want to start with the more affordable option (semaglutide at $299/month)

  • You're seeking 15-18% weight loss with solid cardiovascular benefit

Consider tirzepatide if:

  • You have more aggressive weight loss goals (20%+ reduction)

  • You have type 2 diabetes or significant cardiometabolic risk

  • Cost is not a primary barrier

  • You tolerate GI side effects well or are willing to manage them

Many patients find success on either medication. What matters most is consistent use, adequate protein intake, resistance exercise, and close medical monitoring.

Personalized Formulations

HOW HARTWATER COMPOUNDS GLP-1

Most GLP-1 weight loss programs offer the same medication every patient gets at every other clinic. At Hartwater Wellness, we work with licensed compounding pharmacies to prepare personalized semaglutide and tirzepatide formulations that include synergistic additives selected by your physician based on your clinical profile. The result is a medication tailored to support your body during the weight loss journey, not just one that suppresses appetite.

Why Personalization Matters

GLP-1 therapy has well-known side effects, the most common being nausea, particularly during the first 4 to 8 weeks of dose titration. GLP-1 patients also eat substantially less, which can affect micronutrient intake at exactly the time the body is undergoing significant metabolic change. Branded GLP-1 medications (Ozempic, Wegovy, Mounjaro, Zepbound) contain only the active ingredient. They do not address these secondary clinical considerations directly. Compounded formulations, prepared by licensed pharmacies for individual patients, allow the addition of clinically chosen vitamins and amino acids that may help patients tolerate the medication better and feel better during weight loss. Our medical director selects additives based on each patient's history, side effect profile, and goals. The personalization is documented in your medical record and reviewed with you at every visit.

Our Compounded Formulations and Their Additives

We offer compounded semaglutide and tirzepatide in four primary formulation variants. Your provider will recommend the right one for you based on your goals, medical history, and tolerance to GLP-1 therapy.

Semaglutide with Vitamin B6 (Pyridoxine)

Vitamin B6 has decades of clinical evidence for reducing nausea. It is a standard component of pregnancy-related nausea treatment and is used in chemotherapy-induced nausea protocols. Adding pyridoxine to compounded semaglutide may help patients tolerate the titration phase more comfortably, particularly during the first 8 weeks when nausea is most likely.

  • Recommended for: patients new to GLP-1 therapy, patients with a history of nausea on prior weight loss medications, patients sensitive to medication side effects.

Semaglutide with Vitamin B12 or L-Carnitine

Vitamin B12 (cyanocobalamin) is a methylation cofactor that supports energy production and red blood cell health. L-Carnitine is an amino acid derivative that transports long-chain fatty acids into mitochondria for beta-oxidation, the cellular process that burns fat. Both additives address common patient experiences during GLP-1 therapy: reduced energy from caloric deficit, and the desire to support active fat metabolism.

  • Recommended for: patients who report fatigue during caloric deficit, athletes and active adults, patients optimizing body composition alongside weight loss.

Tirzepatide with Methylcobalamin (Methylated B12)

Methylcobalamin is the bioactive form of Vitamin B12. Unlike cyanocobalamin, which the body must first convert, methylcobalamin is immediately available to support methylation pathways, neural health, and cellular energy. For patients on tirzepatide, which produces a more pronounced caloric deficit due to its dual GIP/GLP-1 mechanism, methylated B12 provides upgraded energy and cognitive support during the weight loss phase.

  • Recommended for: patients on tirzepatide seeking maximum energy support, patients with documented methylation challenges (MTHFR variants), patients prioritizing cognitive clarity during weight loss.

Tirzepatide with Vitamin B12 or L-Carnitine

For tirzepatide patients who do not need the premium of methylated B12, standard cyanocobalamin provides the same baseline energy and methylation support. L-Carnitine offers an alternative for patients focused specifically on fat oxidation. Your provider will help you choose based on your priorities and budget.

  • Recommended for: tirzepatide patients optimizing for cost, patients pursuing maximal fat oxidation support, patients with no specific methylation concerns.

Compounded vs. Branded: Your Options at Hartwater

Patients sometimes ask whether compounded GLP-1 is the right choice for them, or whether they should pursue the branded versions. At Hartwater, we believe in giving you both options and the information to choose well.

Our Compounded Program (Primary Offering)

Personalized semaglutide and tirzepatide formulations with the additives described above, prepared by licensed Texas 503A compounding pharmacies. Pricing is a transparent flat rate: $299 per month for compounded semaglutide and $499 per month for compounded tirzepatide, regardless of dose. Includes all provider visits, lab monitoring, body composition tracking, home delivery, and topical numbing cream.

Important: Compounded medications are not FDA-evaluated for safety or efficacy. Compounding is regulated under FD&C Act Section 503A; our partner pharmacies maintain Texas state licensure and meet USP <797> sterile compounding standards.

Branded FDA-Approved Option (Alternative)

For patients who prefer FDA-approved branded medications, we are happy to prescribe Ozempic®, Wegovy®, Mounjaro®, or Zepbound® at manufacturer pricing. Branded retail pricing typically runs $800 to $1,400 per month depending on the medication and pharmacy. Insurance coverage varies; we will work with you to understand your options. The clinical program (provider visits, lab monitoring, body composition tracking) is identical.

How to Choose

Most of our patients select the compounded program for its combination of accessible pricing, personalization, and clinical oversight. Patients who prefer the regulatory assurance of FDA-approved manufacturing, or who have insurance coverage for branded GLP-1, may prefer the branded option. Your provider will discuss the trade-offs with you during your consultation and help you decide based on your specific circumstances.

A Physician-Led Protocol

GLP-1 THERAPY AT HARTWATER WELLNESS

Lab-Guided GLP-1 Weight Loss Protocols

Hartwater Wellness is not a prescription vending machine. We believe GLP-1 therapy is a serious medical intervention that requires baseline assessment, ongoing monitoring, and personalized optimization. Our protocol integrates laboratory medicine, body composition analysis, and metabolic coaching to maximize safety and outcomes.

Before starting GLP-1 therapy, we recommend a comprehensive metabolic workup that includes:

  • Fasting glucose and insulin: Establishes baseline glucose control and HOMA-IR (insulin resistance index)

  • HbA1c: 3-month glucose average; predicts diabetes risk and monitors long-term glycemic control

  • Lipid panel: Total cholesterol, LDL, HDL, triglycerides

  • Liver and kidney function: AST, ALT, creatinine, eGFR; critical safety baseline

  • Inflammatory markers: hsCRP, optional adiponectin, leptin

  • TSH and free T4: Rules out thyroid dysfunction; GLP-1 can affect thyroid hormone absorption

  • Body composition analysis: InBody Scan quantifies baseline fat mass, lean mass, and visceral adiposity

  • Blood pressure and resting heart rate: Baseline cardiovascular metrics

We also screen for contraindications: personal or strong family history of medullary thyroid cancer (MTC), multiple endocrine neoplasia type 2 (MEN2), severe pancreatitis history, or uncontrolled psychiatric illness. These are absolute or relative contraindications to GLP-1 therapy.

Hartwater vs. Telehealth GLP-1 Programs

National telehealth platforms have made GLP-1 medications more accessible than ever, often at lower price points than local clinics. For many patients, telehealth is a reasonable starting point. But GLP-1 is a long-term metabolic intervention with real clinical considerations, and the differences in care depth between telehealth and a local physician-led program are substantial. Here is how the two compare.

Titration, Monitoring & Protocol Adjustment

Both semaglutide and tirzepatide require slow titration to minimize GI side effects and allow your body to adapt. Here's what to expect:

Typical Semaglutide Titration:

  1. Weeks 1-4: 0.25 mg weekly (lowest dose)

  2. Weeks 5-8: 0.5 mg weekly

  3. Weeks 9-12: 1.0 mg weekly

  4. Weeks 13-16: 1.7 mg weekly

  5. Week 17+: 2.4 mg weekly (maintenance)

Typical Tirzepatide Titration:

  1. Weeks 1-4: 2.5 mg weekly

  2. Weeks 5-8: 5 mg weekly

  3. Weeks 9-12: 7.5 mg weekly

  4. Weeks 13-16: 10 mg weekly

  5. Weeks 17-20: 12.5 mg weekly

  6. Week 21+: 15 mg weekly (maintenance)

You'll have a nurse check-in at each dose adjustment to monitor tolerability, appetite suppression, weight change, and side effects. If you experience significant nausea or GI distress, we may pause titration for an extra week or extend the dose interval.

After reaching maintenance dose, you'll schedule monthly in-person visits for the first three months, then transition to quarterly appointments (or more frequently if adjusting other therapies). At each visit:

  • Weight and body composition measurements

  • Blood pressure and heart rate

  • Appetite and appetite suppression assessment

  • GI tolerance and other side effects

  • Medication adherence and injection technique

  • Protein intake and exercise consistency

Laboratory monitoring occurs at Month 3 (comprehensive panel), Month 6, and Month 12, then annually. We track the same markers as baseline to quantify metabolic improvements.

Body Composition Tracking With InBody 970s

Weight loss is important, but body composition is everything. On GLP-1 therapy with adequate protein and resistance exercise, you should preferentially lose fat while preserving or even gaining lean muscle mass. This preserves your metabolic rate and functional capacity.

We use the InBody 970s body composition scanner, the most advanced bio-impedance device available to track:

  • Total fat mass loss

  • Lean body mass (muscle) preservation

  • Visceral fat reduction

  • Bone mineral density

On average, patients on GLP-1 with proper nutrition and exercise lose 3-4 lbs of fat per month while maintaining lean mass. This is fundamentally different from severe caloric restriction, which strips both fat and muscle.

GLP-1 as Part of Your Metabolic Optimization Stack

While GLP-1 is a powerful tool, it works best as part of an integrated metabolic optimization plan. At Hartwater Wellness, we often combine GLP-1 therapy with:

  • Protein-First Nutrition Plan: 1.0-1.2 grams per pound of body weight daily to preserve lean muscle during weight loss. We provide personalized macronutrient targets and work with you on meal timing and satiety cues.

  • Resistance Training Protocol: 3-4 sessions per week of strength training stimulates muscle protein synthesis and offsets GLP-1's reduction in protein synthesis rates. Our coaches provide programming and form feedback.

  • IV Amino Acid Therapy: Intravenous amino acid infusions (e.g., BCAAs) during caloric restriction can amplify muscle preservation, especially when combined with strength training. This is optional but highly effective for those with aggressive body composition goals.

  • Peptide Therapy for Muscle & Recovery: Growth hormone-releasing peptides (GHRPs), TB-500, or BPC-157 can accelerate recovery and support muscle hypertrophy during GLP-1-mediated weight loss. These are prescribed selectively and monitored closely.

  • Hyperbaric Oxygen Therapy (HBOT): Improves wound healing, reduces inflammation, and may enhance mitochondrial function during metabolic remodeling. Offered as an adjunct for patients seeking maximal recovery optimization.

WHO QUALIFIES FOR GLP-1 THERAPY?

GLP-1 medications are FDA-approved for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI 27-29.9) with at least one weight-related comorbidity. However, appropriate use extends beyond FDA label indications.

Clinical Criteria for GLP-1 Prescribing

You're a candidate for GLP-1 therapy if you meet one of the following:

  1. BMI ≥ 30 (obese category)

  2. BMI 27-29.9 with at least one comorbidity: hypertension, dyslipidemia, type 2 diabetes, prediabetes, sleep apnea, cardiovascular disease, or NAFLD

  3. Type 2 diabetes with inadequate glycemic control

  4. High cardiovascular risk (history of MI, stroke, or significant atherosclerosis) even without obesity

  5. Metabolic dysfunction: insulin resistance (HOMA-IR >2.5), dyslipidemia, or impaired fasting glucose

  6. Prior failed weight loss attempts with lifestyle modification alone

  7. Body composition optimization in metabolically healthy individuals with high performance goals

When GLP-1 Is NOT the Right First Step

  • Personal or strong family history of medullary thyroid cancer (MTC) or MEN2

  • Pancreatitis history (relative contraindication; case-by-case assessment)

  • Severe uncontrolled depression, anxiety, or eating disorders (risk of worsening)

  • Pregnancy or intent to become pregnant (GLP-1 safety in pregnancy not established; recommend 2-month washout)

  • Severe gastroparesis or chronic intestinal pseudo-obstruction

  • Active thyroid disease or uncontrolled thyroid dysfunction

  • eGFR <15 mL/min/1.73m² (severe renal impairment; relative contraindication)

  • Uncontrolled type 1 diabetes (risk of DKA)

If you fall into any of these categories, we'll work with you to optimize other avenues: targeted nutrition, exercise, bioidentical hormone optimization, metabolic medication (metformin, GLP-1 alternatives), or referral to specialists.

THE FIRST 90 DAYS

WHAT TO EXPECT ON GLP-1s

Here's a week-by-week and month-by-month overview of your GLP-1 journey at Hartwater:

Week 1-4 (Initial Titration Phase):

  • You receive your first injection (0.25 mg semaglutide or 2.5 mg tirzepatide) and begin taking detailed appetite, energy, and GI notes

  • Hunger suppression typically becomes noticeable by Day 2-7; most patients report feeling satisfied with smaller portions

  • GI effects (nausea, mild diarrhea) occur in ~40% of patients in Week 1-2; these usually subside with hydration and smaller meals

  • Minimal weight loss in Week 1-4 (mainly water weight); the appetite suppression is the real signal that the medication is working

  • We call at Day 3-5 to check in; many patients adjust meal size or meal timing based on their response

Week 5-8 (Second Dose Escalation):

  • You increase to the next dose tier and experience deeper appetite suppression

  • Weight loss typically becomes measurable: 2-4 lbs per week at this phase

  • GI effects usually peak at dose escalation and improve within days

  • Month 1 lab assessment (optional): repeat glucose, lipids if baseline was abnormal

Week 9-12 (Third Escalation, Early Momentum):

  • Month 2-3: you're now titrating toward therapeutic dose; appetite suppression reaches peak effect

  • Weekly weight loss continues at 2-3 lbs; cumulative loss is 6-12 lbs by end of Week 12

  • Month 3 in-person visit: comprehensive lab panel (glucose, lipids, liver, kidney, inflammatory markers), body composition scan, and progress photos

  • Most patients report improved energy, better sleep, and less joint pain by Month 3

Beyond Month 3: You transition to monthly visits, then quarterly. Weight loss typically accelerates Month 4-6 as you approach therapeutic dose, then plateaus Month 7-12. Expected total weight loss at 12 months: 15-22% body weight.

Important Information About Compounded Medications

Hartwater Wellness's primary GLP-1 offering uses compounded semaglutide and tirzepatide formulations prepared by licensed Texas 503A compounding pharmacies. We want every patient to understand what this means.

What is compounding?

Compounding is the practice of preparing a customized medication for an individual patient based on a physician's prescription. Compounded medications have been used in medicine for centuries and are regulated under federal law (FD&C Act Section 503A) and by state pharmacy boards. The Texas State Board of Pharmacy licenses and oversees compounding pharmacies in our state.

Why does Hartwater offer compounded GLP-1 medications?

Compounded formulations allow us to add clinically chosen vitamins and amino acids (such as Vitamin B6 to reduce nausea or Vitamin B12 to support energy) that are not available in branded products. The personalization is documented in your medical record. Compounded medications are also typically more accessible in cost than branded alternatives.

Important safety information.

Compounded medications are not evaluated by the FDA for safety, efficacy, or quality in the same way that branded medications are. They are not generic versions of branded medications. They are individually prepared formulations that may differ in concentration, additives, or other characteristics. Our partner pharmacies maintain current Texas licensure and meet USP <797> sterile compounding standards, but compounding is inherently a different regulatory framework than FDA-approved manufacturing.

Branded FDA-approved alternatives are available.

If you prefer the regulatory assurance of FDA-approved branded medications, we are happy to prescribe Ozempic®, Wegovy®, Mounjaro®, or Zepbound® at manufacturer pricing. Your provider will discuss the trade-offs with you so you can choose the right path for your situation.

Questions?

Your provider will answer any questions about our compounding program during your consultation. We believe in transparent, informed consent and want every patient to feel confident in their choice.
